1. Clinical impression

High-risk ischemic chest pain based on symptom pattern and cardiovascular risk factors. The demo prioritizes acute coronary syndrome (ACS) while keeping other dangerous causes in view.

2. Important differentials

  • Acute coronary syndrome — highest priority
  • Aortic dissection — consider if tearing pain, pulse/BP differential or neurological features
  • Pulmonary embolism — assess risk factors and associated symptoms
  • Pneumothorax — consider with unilateral pleuritic pain and reduced breath sounds

3. Immediate investigations

  • 12-lead ECG as soon as possible
  • Serial high-sensitivity cardiac troponin where available
  • Focused examination and repeat vital signs
  • Targeted tests guided by the differential and local availability

4. Low-resource pathway

If advanced testing is unavailable, prioritize clinical assessment, serial ECGs/vitals, referral or transfer to the nearest appropriate facility, and clear documentation of deterioration triggers.
